In consideration of the acceptance and processing of my application for employment, and of my employment with ILLIANA FINANCIAL, if I am employed, I warrant and represent as follows:

CERTIFICATION. I certify that I have not knowingly withheld any information that might adversely affect my chances for employment with ILLIANA FINANCIAL and that the information given by me is true and correct without any mental reservation whatsoever. I understand that any omission or misstatement of material fact on my application for employment, resume or any other document used to secure employment with ILLIANA FINANCIAL, or during any pre-employment interview, shall be grounds for rejection of my application or for immediate discharge if I am hired, regardless of the time elapsed before discovery.
REFERENCE AND BACKGROUND CHECK. I authorize ILLIANA FINANCIAL to thoroughly investigate my references, work record, education, criminal record, and any other matters related to my suitability for employment. I also authorize my former employers to disclose to ILLIANA FINANCIAL any and all of my employment records, including disciplinary reports and letters of reprimand, without giving me notice of such disclosure. I hereby fully release and discharge ILLIANA FINANCIAL, my former employers, their respective shareholders, directors, officers, employees and agents, and all other persons and entities, from any and all claims, demands and liabilities arising out of or in any way related to such investigation or disclosure.
DRUG TESTING. I understand that if I am determined to be otherwise qualified for employment with ILLIANA FINANCIAL I may be required to submit to a post-offer drug test as a condition of hiring. I also understand that refusing to submit to such test or testing positive for illegal drugs will be grounds for rejection of my application or for withdrawal of any conditional job offer that may be made to me. I hereby fully release and discharge ILLIANA FINANCIAL, its member-owners, directors, officers, employees, physicians, technicians and agents, and all other persons and entities, from any and all claims, demands and liabilities arising out of or in any way related to any such drug test, the conduct or administration of such test, or the use or disclosure of the results thereof.
FAIR CREDIT REPORTING ACT. I hereby authorize ILLIANA FINANCIAL to obtain a consumer report from a consumer reporting agencies of ILLIANA FINANCIAL'S choice for any employment purpose. In accordance with the Fair Credit Reporting Act, I understand that should information be obtained from a consumer report which may be used for an adverse employment action based on whole or in part on the consumer report, before taking any adverse action, ILLIANA FINANCIAL shall provide the undersigned's rights under the Fair Credit Reporting Act. This release is provided with the understanding that the information is for the use of ILLIANA FINANCIAL. I hereby release you as the requestor and user of such records, the organizations which originated them, or any individual related to such organizations, both individually and collectively, from any liability or legal responsibility that may arise from this Authorization to Obtain Consumer report.
AT-WILL EMPLOYMENT. If I am hired by ILLIANA FINANCIAL, I must be approved and accepted for bonding by a surety company designated by the Credit Union, and agree, in consideration of my employment, to abide by the policies, procedures and rules, or other management communications, as may be directed to employees of ILLIANA FINANCIAL. I understand and agree that if I am hired; my employment and compensation are for no definite or determinable period and may be terminated at any time, with or without cause, warning or notice, and the option of either myself or ILLIANA FINANCIAL. I further understand and agree that no representative of ILLIANA FINANCIAL is authorized to make any binding representations or agreements, whether oral or written, contrary to the foregoing or purporting to guarantee any particular benefits, terms, or conditions of employment.
RE-APPLICATION POLICY. I understand that under ILLIANA FINANCIALS' policy, this application will remain active for only sixty (60) days. Therefore, if I have not been hired within sixty (60) days of the date of this application and I still wish to be considered for employment with ILLIANA FINANCIAL, I understand that I will need to complete and submit a new employment application.
